Below are instructions for medical practitioners and hospitals or clinics that wish to apply to the Abortion Supervisory Committee for certification or a licence.

Certifying consultants

Medical practitioners wishing to be appointed as certifying consultants need to complete the application form below and submit it to the Committee.

In support of your application you will need to include an Annual Practising Certificate, evidence of vocational registration and two written references.

The Committee must consult with the New Zealand Medical Association before the appointment of any medical practitioner as a Certifying Consultant.

Institutions

Hospitals or clinics wishing to apply for a licence to perform abortions must complete the application form below and submit it to the Committee.
